Individual
Use this form when nominating an individual practitioner for an award.

Before you begin
Please make sure you have the following ready:
- Full contact details for both the nominator and the nominee
- A clear understanding of the award category being entered
- Quantified examples of the nominee's impact (numbers, metrics, before/after data)
- Two (2) testimonials — at least one from outside the nominee's organization
- Professional CV or organizational profile of the nominee
- Supporting documents: case studies, published work, dashboards, or other evidence
Accepted upload formats: PDF, DOCX, JPG, or PNG · max 25 MB each. Online submissions only — email, hand-delivered, and postal nominations are not accepted.
